Appendix H helps participants who want to live with non-SDA funded family members or friends, providing additional funding to cover housing costs in shared arrangements. This allows participants to enjoy familiar company, maintain independence, and still access the benefits of SDA housing.
Why this matters for participants:
Live with family or friends while keeping independence
Enjoy familiar surroundings and social support
More flexibility in choosing housing options that suit their lifestyle
For Support Coordinators:
Appendix H allows participants to receive additional funding when living with non-SDA funded family members or friends. This gives you more flexibility in planning housing arrangements that suit the participant’s social and emotional needs.
You can:
Help participants maintain independence while enjoying familiar company.
Help participants find housing that supports their wellbeing and reduces daily stress.
Ensure participants have access to safe, high-quality SDA homes with extra support for shared arrangements.
For SIL Providers:
Appendix H provides funding support that covers additional costs in shared living arrangements.
This means you can:
Offer participants more flexible housing options without compromising on care quality.
Support participants in living with family or friends, creating a more familiar and supportive environment.
Ensure smooth coordination with SDA Provider while maintaining reliable funding for care services.
We are happy to share that several of our SDA homes are eligible for Appendix H arrangements. Each home is purpose-built, fully accessible, and designed to provide safe, high-quality living for participants in shared or family-inclusive environments.
